Attention: ALL 1st YEAR, 2nd YEAR, and PC STUDENTS

You are REQUIRED to update your personal information records ASAP. You may download the Updating Form below or get printed copies from the Registrar’s Office

Please submit the accomplished Updating Form to the Registrar’s Office on or before May 7, 2016 in order to be cleared from the End of Semester Clearance.

Please be informed that you need to be CLEARED by all the following offices for you to be allowed to participate in the graduation activities:

• Admissions Office
• ADSA Office
• Administrative Office
• Campus Ministry Office
• Clinic
• First Year Coordinator Office
• Guidance Center
• Laboratory
• Language Learning Center
• Records Section
• Sanggunian Office
• Library
• Student Accounts Section

A list of students who are not yet cleared (and which office/s they still have deficiencies) will be posted on May 3, 2016 (Tuesday) at the Registrar’s Office and ADSA Office. If your name is there you need to settle your deficiencies and submit your printed clearance (indicating you are cleared in all the offices) to Ms. Gelai at the ADSA Office on or before May 12, 2016 (Thursday)

FAILURE TO DO SO will result to your EXCLUSION from the list of graduating students who can join the Baccalaureate Mass and Recognition Ceremony (May 19) and Graduation Rites (May 21). In this regard, you will not also be included in the graduation practices which will start on May 16.

All students are required to evaluate teachers using the Student Evaluation of the Learning Situation (SELS). Kindly accomplish the evaluation to be cleared from the End of Semester Clearance. Non- accomplishment can cause student to be "on-hold" for the succeeding enrollment.
