Documents

Submit the documents below to the Admissions Office on or before the deadline.

Incoming First Year Students (Filipino)
  1. Form 138 or Original Grade 12 Report Card

  2. Original PSA Birth Certificate (for Filipino citizens and those born outside the Philippines with Filipino parents)

  3. Photocopy of Baptismal/Dedication Certificate or other document that may certify your religion. (First Communion certificate is not acceptable)

Undergraduate Transfer Students (Filipino)
  1. Honorable Dismissal
  2. Original PSA Birth Certificate (for Filipino citizens and those born outside the Philippines with Filipino parents)
  3. Photocopy of Baptismal/Dedication Certificate or other document that may certify your religion
  4. Crediting and Advising 
  5. Official Transcript of Records or Certified True Copy of Grades and course descriptions of general education subjects taken (with grades at least 87% or its equivalent)
Undergraduate Foreign Students
  1. 2 (two) copies of Original Transcript of Records with English translation and duly authenticated by the Philippine Foreign Service Establishment located at the student’s country of origin or legal residence
  2. Present original copy of Birth certificate/ Family Registry and submit photocopy
  3. Documents for student visa conversion (9F) or special study permit (SSP) application.
  4. Visit the Admissions Office for additional requirements
Graduate Program Students (Filipino)
  1. Original PSA Birth Certificate
  2. Original PSA Marriage Certificate (for married applicants)
  3. Medical Certificate certifying that you are FIT TO STUDY
  4. Transfer Credential or Honorable Dismissal
  5. Student Online and Registration System ACCESS FORM
Graduate Program Students (Foreign)
  1. Original PSA Birth Certificate
  2. Original PSA Marriage Certificate (for married applicants)
  3. Medical Certificate certifying that you are FIT TO STUDY

Kindly go to the College Clinic to set an appointment for your medical examination. Please see the available dates on your Acceptance Letter.

On the day of your medical examination, please wear loose-fitting clothes and bring the following:

Location: MAE Tech-Voc Building
Use the school's main gate (Gate 1) and turn right into the College Parking lot. The MAE Tech-Voc Building is at the far end of the parking lot. It also houses the Kulay Salon.
