Submit the documents below to the Admissions Office on or before the deadline.

Incoming First Year Students (Filipino)
  1. Form 138 or Original Grade 12 Report Card

  2. Original PSA Birth Certificate (for Filipino citizens and those born outside the Philippines with Filipino parents

Undergraduate Transfer Students (Filipino)
  1. Transfer Credential or Honorable Dismissal
  2. Original PSA Birth Certificate (for Filipino citizens and those born outside the Philippines with Filipino parents)
  3. Crediting and Advising 
  4. Official Transcript of Records or Certified True Copy of Grades and course descriptions of general education subjects taken (with grades at least 87% or its equivalent)
Undergraduate Foreign Students
  1. 2 (two) copies of Original Transcript of Records with English translation and duly authenticated by the Philippine Foreign Service Establishment located at the student’s country of origin or legal residence
  2. Present original copy of Birth certificate/ Family Registry and submit photocopy
  3. Documents for student visa conversion (9F) or special study permit (SSP) application.
  4. STUDENT VISA and photocopy of the following pages:\

    • Bio-page
    • Entries of the latest arrival date
    • authorized stay
  5. Visit the Admissions Office for additional requirements

Scholarship and Financial Aid

Medical Examination

Kindly go to the College Clinic to set an appointment for your medical examination. Please see the available dates on your Acceptance Letter.

On the day of your medical examination, please wear loose-fitting clothes and bring the following:

  2. 1 piece 1” x 1” ID picture
