Request for School Documents

Students and alumni may request for school documents such as diploma, TOR, certificates, and authenticaed documents from the Registrar's Office. To know the types and cost of documents, please view the Request Form below:

  1. Download and accomplish the Registrar's Request Form.
  2. Email the accomplished form to the HEU Registrar's Office.
  3. Pay the document fee/s at any of the following:
  • BPI
  • BDO

BPI- Bills Payment (Over the Counter / online banking)

  1. Email a clear copy of the (1) deposit slip or confirmation page with your (2) Name and Student ID No. to college_registrar@mc.edu.ph; mctreasury@mc.edu.ph; mcaccounting@mc.edu.ph ; csinson@mc.edu.ph for validation of payment. 
  2. You will receive a acknowledgment of payment and instructions on how you may claim your document. 
  3. You or an authorized representative may claim the document from the the Registrar's Office. Your representative must present his/her ID, your signed Authorization Letter and a photocopy or scanned copy of your ID.

Offices may be reached via email and mobile number from Monday to Friday 8:00 AM to 5:00 PM. The trunkline and its local numbers may be reached during the onsite schedule from Tuesdays to Thursdays, 9:00 AM to 4:00 PM.

Visits to offices are by email appointment and follow the onsite schedule.


Request for Recommendation Letter

Students and alumni may request for recommendation letters from the HEU Guidance & Counseling Office. Recommendation letters are usually required for on-the-job trainings and application for a job or new school. 

To request for recommendation letters, follow the instructions below:

  1. Download and accomplish the Recommendation Letter Request Form.
  2. Email the accomplished form to the HEU Guidance & Counseling Office emails mcheuguidance@mc.edu.ph.
  3. Pay the processing fee at:
