What's Next

Once you have submitted all your application requirements, kindly wait for the Admissions Office to email you an update regarding your application status. If you have not received any emails within 3 working days, you may contact the Admissions Office to follow-up on your status.

Documents

Submit the documents below to the Admissions Office on or before the specified deadline:

  1. Original Hard copies of all application requirements:

    • Application Form (print from SDMS)
    • 2 Recommendation Forms
    • Original and photocopy of Transcript of Records of the last school attended
    • Curriculum Vitae
    • Personal essay on one’s strengths, achievements, and goals
       

  2. Original Philippine Statistics Office (PSA) Birth Certificate

  3. Original Philippine Statistics Office (PSA) Marriage Certificate, if married

  4. For Transferees, Transfer Credential or Honorable Dismissal

  5. For Foreign applicants, who studied in schools outside the Philippines, submit hard copies of the following items:

    • Original Transcript of Records with English translation (or translated in English) duly authenticated or apostille stamped by the Philippine Foreign Service Post/Establishment located at the student’s country of origin
    • Certificate of Birth/Family Register
    • Photocopy of passport:
      • Bio-page
      • Entries of the latest arrival date
      • Authorized stay
    • Student Visa (Admissions Office will assist in applying for Student visa for foreign applicants residing in the Philippines)
