How to Apply

During lockdown period, you may follow the steps below to begin your application

Application Procedure

  1. Fill-out the online Information Sheet >
  2. For New Students, send scanned copy of the following to sdtec-admissions@mc.edu.ph
    • PSA Birth Certificate
    • Form 138 - Report Card (If available, otherwise, it may be sent later)
    • Recent passport size photo
  3. Pay over the counter or via online payment.
    • Below are modes of payment:
BPI / GCASH

BPI Transfer Money (thru online) or Regular Deposit (over the counter)

Payment may also be made through GCash – Send Money / Bank Transfer using the same account number.

BDO / GCASH

BDO Deposit Referencing (over the counter) or Bills Payment (thru online)

  1. Email clear copy of the validated deposit slip of payment with the name of the student to:
    • mctreasury@mc.edu.ph
    • sdtec-admissions@mc.edu.ph
    • mcaccounting@mc.edu.ph
  2. Receive Acknowledgement and the details of the Official Receipt (OR) from the Treasury Office.
  3. Present original documents together with the Acknowledgement Email with OR details to the Office of Academic Support and Student Services for confirmation once offices reopen.

Admission Requirements:

  • PSA Birth Certificate (original)

  • Report Card (Form 138) – original

  • Transcript of Records (TOR/Form 137)

  • Character Reference from last school attended / sponsoring organization or Barangay/Police Clearance

  • Recommendation Form from the Guidance Counselor

  • ITR or Certification with salary from employer (if applicant is employed) / ITR or Certification with salary of parents (if applicant is unemployed)

  • Photocopy of PSA Marriage Certificate (if married)

  • Passport size pictures (white background with name)

  • Chest x-ray (film and result) – to be submitted during enrollment

  • Junior High School Certificate (for SHS students)

  • QVA Certificate (for SHS students from private school)
