Documents

Submit the documents below to the Admissions Office on or before the deadline.

Incoming First Year Students
  1. Original 4th Year High School Report Card

  2. Original NSO Birth Certificate (for Filipino citizens and those born outside the Philippines with Filipino parents)

  3. Photocopy of Baptismal/Dedication Certificate or other document that may certify your religion. (First Communion certificate is not acceptable)

  4. Conforme for those with On Probation Admission Status

Transfer Students
  1. Honorable Dismissal
  2. Original NSO Birth Certificate (for Filipino citizens and those born outside the Philippines with Filipino parents)
  3. Photocopy of Baptismal/Dedication Certificate or other document that may certify your religion
  4. Conforme for those with On Probation Admission Status
  5. Crediting and Advising 
  6. Official Transcript of Records or Certified True Copy of Grades and course descriptions of general education subjects taken (with grades at least 87% or its equivalent)
Foreign Students
  1. 2 copies of Original Transcript of Records with English translation and duly authenticated by the Philippine Foreign Service Establishment located at the student’s country of origin or legal residence
  2. Notarized Affidavit of support including bank statement
  3. Present original copy of Birth certificate/ Family Registry and submit photocopy
  4. Documents for student visa conversion (9F) or special study permit (SSP) application. Please see Ms. Catherine Collantes of the Admissions Office for the complete list.

On the day of your medical examination, please wear loose-fitting clothes and bring the following:
