Undergraduate Transfer Students (Filipino)
  1. Copy of Grades/Transcript of Records
  2. Recommendation Form from Professor
  3. Recommendation Form from Guidance Counselor

Note: Crediting and Advising will be processed after reservation.

Undergraduate Foreign Students
  1. Scanned Copy of Passport Biopage and VISA
  2. Copy of latest departure and arrival stamp in your passport
  3. Recommendation Form from Class Adviser
  4. Recommendation Form from Guidance Counselor
  5. Transcript of Records

Scholarship and Financial Aid
